They collaborate with health, education, and case management teams to help families overcome barriers and achieve their goals. Requires post-secondary education in Social Work, Human Services, or a related field and 2-3 years of experience in service navigation or community programs. Candidates must have a valid driver's license and a deep understanding of Indigenous cultures and trauma-informed care.
