This role is patient centered, flexible, cost effective, and quality driven, focusing on advocating for clients and coordinating resources based on assessed needs. Working in Primary Health Care, the position serves as an entry point for accessing services such as community, home, long term, and acute care. Key duties include intake, engagement, care planning, monitoring, evaluation, and discharge. The role collaborates with healthcare providers, families, caregivers, and agencies to assess needs, coordinate services, and ensure timely, appropriate care delivery. Human Resources Exemption: No Education * Bachelor Degree in a related Health Science or Social Science discipline Knowledge and Abilities * Ability to manage a high demand caseload and other work assignments. * Ability to facilitate applications/participate in integrative case management * Ability to appropriately intervene/attain intervention in emergency situations * Demonstrated knowledge and ability for competent case management practice * Ability to develop educational resources and presentations * Sound clinical judgement skills. * Cost conscious, quality driven care planning and service delivery * Demonstrates ability to manage time and competing deadlines * Ability to establish and maintain interpersonal/consultative relationships * Advocacy, negotiation, crisis management and conflict resolution * Has demonstrated strong interpersonal and communication skills. * Demonstrates empathy, trust and respect in the provision of care * Knowledge of social determinants of health affecting vulnerable populations * Commitment to quality, safety, and improvement striving towards zero harm * knowledge of information privacy principles (LA FOIP and HIPA) * Adherence to the Code of Ethics. * Responsible for accurate observation/documentation of patient related info.
