The Resident Services Coordinator supports prospective residents and their families through the admission process. This includes responding to inquiries, conducting tours of the home, reviewing applications, coordinating pre-admission visits and assessments, and working closely with placement agencies and Ontario Health at Home. The role ensures clear communication, smooth transitions, and a positive experience for resident and families entering long-term care. Key Responsibilities: Respond to inquiries about new resident placement and provide tours for prospective residents. Manage pre-admission procedures, including reviewing applications in conjunction with the Social Worker and Director of Care, communicating with applicants and placement agencies, completing pre-admission visits and assessments, and guiding residents/families through the admission process with Ontario Health at Home. Serve as the primary point of contact for residents and families regarding non-clinical services and general inquiries. Assist residents and families in understanding services, and available supports within the LTC home. Coordinate resident admissions, orientations, and discharges to ensure smooth transitions. Collaborate with interdisciplinary team to support resident needs. Maintain accurate documentation and resident records in compliance with organizational policies and regulatory standards. Facilitate communication between families, residents, and staff to address concerns and enhance satisfaction. Monitor and report resident feedback or concerns to the appropriate care team members. Participate in the Manager on Call rotational schedule. Knowledge and Skills Required: Previous experience in a long-term care, healthcare, or senior living environment strongly preferred. Knowledge of the Fixing Long-Term Care Act and related regulations is an asset. Strong interpersonal and communication skills with the ability to interact compassionately with residents, families, and staff. Excellent organizational, time management, and problem-solving skills. Ability to work collaboratively within a multidisciplinary team. Proficiency with electronic health records and Microsoft Office Suite. Commitment to resident-centered care and continuous quality improvement. Working Conditions: Work performed primarily in resident care areas and office settings.
