The IEN Intern plays a key role in delivering on Unity Health Toronto (UHT)'s mission, vision & values and in advancing UHT's Professional Practice strategic directions.

Participants apply existing nursing knowledge and expertise in a preceptored, fully supervised clinical experience called the Supervised Practice Experience Partnership (SPEP) program. They advance their understanding of interventions that promote health and wellness for a broad range of challenges within a patient safety framework. Participants utilize a process of reflective practice and critical inquiry to evaluate individual and team decision-making processes and actions. Participants assume increasing autonomy and accountability in the provision of care for clients under the supervision of a preceptor. Participants create learning plans that focus on improved problem solving, holistic care, and evidence-based practice that meet the unique and variable health care needs of diverse clients across the continuum of care. The IEN Intern continually enhances his/her practice and competencies through ongoing learning, education, experience and reflective practice.

The primary role of the IEN Intern is to provide holistic, evidence-based, culturally appropriate, patient-centered quality care in a supervised setting in accordance with the College of Nurses of Ontario standards and requirements and UHT's mission, vision, values and policies. This includes:

  • Being accountable for one's decisions and professional practice;
  • Establishing a therapeutic relationship with the patient;
  • Collaborating with other members of the patient's care team; and,
  • Coordinating and communicating patient care activities to optimize the patient's experience and care outcomes.

  • DUTIES & RESPONSIBILITIES:

    The IEN Intern, working together with Preceptor:

  • Acquires relevant health and situational information from a variety of sources to assess individual patient care and learning needs
  • Develops a plan of care to address priorities and achieve desired goals; integrates nursing activities with other inter-professional plan of care activities to facilitate coordinated care
  • Engages in critical inquiry and reflection to select evidence-based interventions to meet patient needs and goals
  • Organizes nursing care based on patient care needs and priorities, assigning or changing nursing care activities where appropriate.
  • Continuously gathers and analyzes clinical and other relevant data to evaluate the effectiveness of the plan of care and adjusts the plan as needed to achieve desired outcomes ensuring inter-professional team members are informed
  • Provides health education using effective teaching strategies that take into account the patient/family's learning needs, readiness, and motivation to learn.
  • Documents observations, assessments and care provision in a timely manner to facilitate a current and accurate record of the patient's status and enable effective inter-professional communication
  • Contributes to discharge and transition planning and other planning activities to facilitate care continuity and coordination; advocates for care needs to be met in most appropriate care setting.
  • Refers patient to other agencies (e.g. CCAC) or services as needed
  • Seeks guidance from preceptor as required
  • Serves as the primary resource for organizing and coordinating patient care activities while working collaboratively with others to achieve agreed-upon outcomes and optimize resource utilization for individual patients
  • Establishes and maintains respectful, collaborative, therapeutic and inter-professional relationships within and across hospital units / programs (and externally where appropriate for his/her patient population)
  • Coordinates and communicates patient care activities to facilitate a safe and seamless transition from one shift or setting/ unit of care to another; ensures appropriate transfer of accountability from one care provider to another
  • Aligns behavior with UHT's vision, mission and values and code of conduct; promotes nursing as a knowledge-based profession; and, supports the advancement of professional practice plans and initiatives
  • Engages in self-reflection, performance review and ongoing learning to maintain and improve professional practice and competence; contributes to others, learning and professional growth
  • Embraces change that leads to improved nursing practice and better patient outcomes
  • Advocates for a safe and healthy work environment
  • Participates in or supports research projects that contribute to nursing and/or inter-professional practice pertaining to his/her unit /program / patient population
  • Supports and participates in the implementation of evidence based practice (best practice guidelines and other forms of evidence) and quality improvement initiatives on unit/in programs including corporate initiatives

  • QUALIFICATONS:

  • Current eligibility letter from the College of Nurses of Ontario indicating eligibility to participate in the Supervised Practice Experience Partnership (SPEP) program.
  • BLS Certification
  • Previous experience required for some programs (unique to each unit/program)
  • Relevant knowledge and/or experience
  • Ability to work collaboratively with other professions / disciplines in a team environment
  • Excellent communication skills
  • Good planning, time management, organization and coordination skills
  • Confidence in oneself and capabilities
  • Commitment to a patient-centred philosophy and inter-professional practice
