Harbour Lights Transitional Care Unit

The Toronto Grace Health Centre (TGHC) provides, specialized care to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Transitional care, and Palliative Care services. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 2 sites – the Main site (Church& Bloor St) and our Harbour Lights site and Specialized Care Centre (Victoria Park & Mc Nicoll). POSITION SUMMARY

As a Community Support Worker, you will work with a team and help co-ordinate various activities that support daily activities of healthy living and facilitates linkages for adults with various mental and psychosocial challenges. The Community Support Worker is an integral part of the team and offer a variety of functions in support of the social and mental wellbeing of clients within a safe and supportive atmosphere.

A key component of this position is enhancing recovery and social recreation/rehabilitation skills of patients that will enable individuals to increase their independence, dignity, and self-respect. The role will require direct service and support to assigned patients as well as group facilitation and programing in areas such as housing, daily living, practical assistance, crisis intervention, linkages to other support services.

MAIN RESPONSIBILTIES

Assist patients with various activities as needed such as: accompanying clients to external appointments, assist with various documentation and phone calls, escort clients on walks and other short trips

Must be willing to accompany patients to external appointments. The appointments should not involve legal or financial matters. Patients may be ambulatory or non- ambulatory (e.g. wheelchairs or mobility aids). This applies to patients who are competent and do not present with unpredictable behaviours.

Help support the effective implementation of various therapeutic programming by working closely with the Interprofessional Practice Team

Working in collaboration with external partners and Social Worker

Participate and support various recreational therapy activities with patients

Build trusting and respectful relationships through interactions and communication

Other related duties as assigned

QUALIFICATIONS & EDUCATION

At least three years of experience working with a diverse client/resident population that supports positive transitions

A College or University Degree in a related field

Ability to work with a diverse client group with various challenges ranging from psychosocial to financial and others

Knowledge of the issues faced by adults with mental health and or addiction or homelessness is an asset

An understanding of how to develop and support therapeutic relationships with assigned clients

Ability to interact, advocate, relate with and negotiate for clients on various issues including goals and priorities in conjunction with a team of interprofessionals

Excellent interpersonal, communication and social skills enabling both team and one on one interactions

Engage the clients in various activities that foster effective transitions that support independent activities of daily living

Ability to deescalate various situations and employ good judgment and conflict resolution/management skills

Good analytical, time management and organizational skills and ability to follow-up on issues or activities with clients

Must have ability to act in accordance with all applicable Policies and Procedures and ensure all interactions maintain the highest level of dignity, respect and professionalism

Knowledge of Microsoft Office Suite

Must maintain highest level of safety, confidentiality and privacy

Excellent attendance, punctuality and work record required

Must have the physical strength, flexibility and endurance required to perform the job, as determined by the physical needs assessment of the position conducted by Occupational Health Services
