Job summary
Woolwich Community Health Centre (WCHC) requires a part-time (0.50 FTE) Family Physician (M.D.) to provide day and evening coverage for our New Hamburg site. The Family Physician functions as a member of the inter-disciplinary team by providing a range of nursing services essential to client care. Requirements: 1. Registered Medical Doctor (M.D.) with current Registration with the College of Physicians and Surgeons of Ontario 2. 2-3 years experience in a community-based primary care setting is preferred; new graduates are welcome to apply. 3. Knowledge of community resources/services to facilitate referrals for primary health care clients 4. Ability to manage multiple and complex demands 5. Commitment to working collaboratively in a team environment 6. Knowledge of/experience with health issues facing seniors, rural residents, youth, and families with young children 7. Familiarity with PS Suite EMR considered an asset 8. Treats conditions and chronic diseases and injury assessment 9. Provide continuous, long-term primary care and patient advocacy 10. Provide acute, preventive, and chronic care 11.Flexibility to work one evening per week as well as provide after hours on-call on shared schedule with 15 other health centre physicians and nurse practitioners. Hours and location of work would be pre-arranged, based on coverage needs at any of the facilities. Requirements
Must be a Registered Medical Doctor with current registration with the College of Physicians and Surgeons of Ontario. Preference is given to candidates with 2-3 years of community-based primary care experience, though new graduates are welcome.
