The Occupational Therapist is accountable for the following: * Assess patients with a holistic perspective: assess physical, psychological, perceptual, cognitive, vocational and functional status through administration of formal and informal assessments for each patient. * Analyze findings to establish patient-centered goals, in collaboration with the patient, families, and interprofessional care team. * Implement a patient-centered plan of care to facilitate/improve daily function * Evaluate progress and goal attainment in the areas identified during the assessment and care planning process. Evaluate the need for supplementary assessments, referrals, equipment/adaptive devices, community services, and education. * Participate in education and discharge planning in collaboration with their team and family to determine patients' functional status and level of independence to facilitate their safe discharge. * Maintain assessment reports and progress notes in the patient's health record. * Collaborate and communicate with interprofessional team and other stakeholder(s) within the circle of care. * Assign and supervise occupational therapy related activities, as appropriate, to Occupational Therapist Assistants (OTA) * Participate in the education of health professions students Qualifications: * Candidate must be a graduate of an accredited School of Occupational Therapy (Master's degree or Bachelor’s Degree in Occupational Therapy.) * Member in good standing with the College of Occupational Therapists of Ontario * Membership in O.S.O.T. and/or C.A.O.T. preferred * Experience treating acute care Adult populations (Medical, Surgical, Neurological, Cardiac Oncology, Palliative and Geriatrics) * Good listening, observation, organization and communication skills are essential. * The ability to work independently, learn quickly and set priorities. * Ability to work collaboratively with an inter-professional team * Satisfactory attendance and performance record required THP is committed to internal talent, and therefore priority will be given to internal applicants. Requirements

Candidates must hold a degree in Occupational Therapy and be a member in good standing with the College of Occupational Therapists of Ontario. Previous experience treating acute care adult populations is required, along with strong communication and organizational skills.
