Support youth or adults with daily living, personal and health care, behavioral, social, educational, cultural, and recreational needs in accordance with person-centered plans and trauma-informed practices. Coordinate with families and community partners, manage household routines and transportation, administer medication, apply crisis and risk-management strategies, and document progress.

Support residents’ health, safety, daily living, and personal goals through individualized care, life-skills training, medication administration, and participation in planned activities. Monitor and document residents’ progress, respond to emergencies, communicate with families and professionals, and maintain the residence and its records.
