The Position Under the direction of Urban Family Services Management and the daily supervision of the Urban Family Services Supervisor, the Family Support Worker provides intensive in-home support services to families involved with or at risk of involvement in the child welfare system. The Family Support Worker develops and implements individualized family support plans to strengthen parenting capacity and family functioning. Services include assessing family needs, providing intervention and case management, facilitating access to community resources, and offering guidance, advocacy, problem-solving, and referrals. The position supports families experiencing challenges related to poverty, addictions, family violence, abuse, and other barriers associated with urban living, with the goal of promoting child safety, family stability, and overall well-being. Responsibilities * Provide services to families who have been deemed to be in need of protection under The Child and Family Services Act, or identified as being in need of interventions and who are at high risk of removal due to issues impacting the safety and well-being of their children. * Provide support and referral services for STC Member First Nations families relocating to an urban setting due to family breakdown. * Provides intensive in-home family support on a weekly basis. * Gathers information relevant to the client’s problems, needs and risks by interviewing, observing behaviour, meeting with caregivers and service providers by using a variety of case management resources. * Develop individual action plans that address client issues, goals, and timelines, unique and strength-based, to identify solutions to overcome barriers and challenges to achieving healthy outcomes. * Provides support, guidance and problem-solving to clients to address issues related to family functioning in an individual or family setting using techniques such as active listening, conflict resolution, basic group counseling techniques and traditional methods to resolve the identified problems, needs and risks. * Maintain accurate client progress and statistics in accordance with funding agreement standards; collect and disseminate statistical data (monthly reports). i.) Input of client data into Client Management System, ii.) Report on key performance indicators and outcome measures. iii.) Prepare quarterly data evaluation to gauge program effectiveness. * Provides supervised visits, plans and supports in the reunification of child/ren. * Participates in the development, modification and evaluation of client service plans with integrated case management team; reports clients’ activities and progress as required. * Provides information on and referral to other community service providers, resources and professionals as required. * Plans and conducts one on one or family educational sessions on topics related to family functioning. * Liaises with and/or promotes the interests of clients with other community service providers, professionals and school personnel as required. Accompany clients to meetings and appointments as required. * Advocate for clients at conference planning meetings, before boards, and courts if applicable. * Adhere to security and confidentiality while maintaining file system of clientele. Education and Experience * Candidates must possess a degree in social work: BISW or BSW, with one-year related experience or diploma in a related human service discipline with a minimum two years related experience. * Experience working with the diverse needs and challenges of urban living. * Efficient in Microsoft Office applications. * Current in First Aid and CPR. Knowledge, Skills and Abilities * Knowledgeable of challenges common to vulnerable families, early childhood development and family dynamics. * Knowledgeable in trauma-informed care principles and practice to children, youth, and families involved with child welfare. * Demonstrated working knowledge of community-based programs and related provincial and community support programs. * Knowledgeable and respective of our multi-cultural society. * Knowledgeable in Indigenous cultures, practices; holistic models of health and wellness * Ability to maintain accurate case management notes, planning, statistics and provide related reports to supervisor as required. * Ability to work collaboratively, demonstrate professional presence, maintain effective working relationships with diverse staff and clientele. * Ability to work with families undergoing high degrees of stress or experiencing crisis. * Ability to work independently and be a supportive team player. * Excellent interpersonal, public speaking, writing, organization and management skills. Other Work Conditions * Must possess a valid Saskatchewan driver’s license, a reliable vehicle and meet STC’s insurance requirements. * Must provide a current, original, Canadian Criminal Record Check (CPIC) with vulnerable sector search as a condition of employment. * Flexible to work after business hours.

Requirements

Candidates must possess a degree in social work with one year of experience or a diploma in a human service discipline with two years of experience. A valid driver's license, reliable vehicle, and a clear criminal record check with a vulnerable sector search are required.
