BAND 6 OCCUPATIONAL THERAPIST – COMMUNITY Location: Sussex Contract: Temporary Our client is seeking an experienced Band 6 Occupational Therapist to join their Unplanned Care Team in Sussex. This role involves providing rapid response and intervention to prevent hospital admissions and support timely discharges, working closely with patients in acute and community settings. KEY RESPONSIBILITIES: * Deliver occupational therapy interventions within an unplanned care service, focusing on admission avoidance and discharge facilitation. * Complete functional assessments and provide equipment recommendations to promote independence. * Work collaboratively with the multidisciplinary team to ensure patient-centered care. * Respond promptly to urgent referrals and manage a varied caseload effectively. REQUIREMENTS: * HCPC registration as an Occupational Therapist. * Experience in acute or community settings, ideally within unplanned care or rapid response services. * Strong understanding of discharge pathways and admission avoidance strategies. * Excellent communication and teamwork skills. * A valid driving licence and access to a car (essential for community visits).
