The Activation Aide is responsible for planning, coordinating, and executing diverse recreational and therapeutic activities to enhance the well-being of long-term care residents. They will collaborate with an interdisciplinary team to implement personalized care plans and maintain accurate documentation of resident participation.

The Social Services Worker collaborates with the inter-professional team to assess resident psychosocial and social needs, developing care plans to promote wellbeing and safety. Key duties include coordinating resident moves, developing social work services, acting as an advocate for residents' rights, and supporting families during planning conversations.
