COMMUNITY PROGRAMS SOCIAL WORKER (BILINGUAL) Make a Difference with Family Services Windsor-Essex (FSWE) We are seeking a Community Programs Social Worker (Bilingual) to join our team. This position provides support across multiple community-based programs serving vulnerable individuals including youth, adults and seniors. The successful candidate will receive training in programming areas including Adult Protective Services, Voluntary Trusteeship Support and Elder Abuse, working collaboratively across a variety of services to meet the changing needs of the agency and the individuals we support. Working from a client-centered, strengths-based approach, the Community Programs Social Worker assists individuals in maintaining independence, navigating complex systems, accessing community resources, and achieving their personal goals while promoting dignity, self-determination, and community inclusion. What You’ll Be Doing Receive referrals and conduct comprehensive assessments to determine client eligibility, needs, strengths, and goals. Develop, implement, and monitor individualized service plans in collaboration with clients. Provide direct community and in-home support to individuals experiencing a variety of social, financial, health, or personal challenges. Support individuals in developing the skills, knowledge, and resources necessary to maintain safe and independent living. Assist clients in navigating healthcare, income support, housing, legal, financial, and other community systems. Provide advocacy and education to help individuals understand their rights, responsibilities, available services, and options. Collaborate with healthcare providers, community agencies, government organizations, family members, caregivers, and other service providers to coordinate supports and improve client outcomes. Identify and facilitate referrals to appropriate community resources and services. Respond professionally and compassionately to complex, sensitive, and crisis-related situations using sound judgment and effective problem-solving skills. Build trusting relationships with clients while encouraging independence, informed decision-making, and goal achievement. Maintain accurate, timely, and confidential client documentation, case notes, and statistical reporting in accordance with agency policies and professional standards. Participate in case reviews, team meetings, and interdisciplinary planning to ensure coordinated service delivery. Maintain knowledge of community resources, relevant legislation, agency policies, and best practices. Participate in ongoing professional development and training to support service delivery across multiple program areas. Perform other related duties as assigned. What You Bring Bachelor of Social Work (BSW) or Master of Social Work (MSW) is required. Fluency in both English and French (written and verbal) is required. Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) as a Social Worker, or eligibility for registration, is preferred. Experience providing case management and support to vulnerable adults, seniors, individuals with disabilities, or those experiencing complex social, financial, health, or housing-related challenges. Strong knowledge of client-centered practice, advocacy, crisis intervention, case management, and community resource navigation. Demonstrated knowledge of community support services, social service systems, and relevant provincial legislation. Excellent assessment, critical thinking, communication, interpersonal, conflict resolution, organizational, and time management skills. Ability to work independently, manage competing priorities, and adapt to changing program needs. Strong documentation and computer skills, including experience maintaining electronic client records. Valid Ontario Class G Driver's License, access to a reliable vehicle, and proof of valid automobile insurance with coverage meeting Ontario's minimum legal requirements. Current Criminal Record Check with Vulnerable Sector Screening. Standard First Aid and CPR certification is considered an asset. Why Join FSWE? Expérience dans le domaine des services de soutien au développement (une expérience auprès de jeunes et d’adultes en situation de handicap ou à faibles revenus constitue un atout). Vous devez être titulaire d’un permis de conduire valide, d’une assurance automobile et disposer de votre propre véhicule. Excellentes compétences en matière de résolution de conflits et de communication. Une formation aux premiers secours constitue un atout. Vos missions Recevoir les demandes de services et réaliser des évaluations des personnes afin de déterminer leur admissibilité au programme. Rencontrer et accompagner les personnes pour les aider à atteindre les objectifs fixés, et assurer un suivi continu de ces objectifs. Travailler avec les personnes pour les aider à acquérir les compétences et les ressources nécessaires pour vivre de manière autonome au sein de la communauté. Accompagner et former les personnes bénéficiant d’un soutien. Travailler en collaboration avec d’autres services communautaires afin d’identifier les besoins des personnes bénéficiant d’un soutien, et les orienter de manière appropriée en respectant les principes d’autodétermination des usagers. Défendre, aux côtés ou au nom des personnes dont les droits à des services ou à des opportunités pourraient être, de fait ou potentiellement, compromis.
