We are recruiting for a Full-time Housing First Case Manager to promote independence and quality of life for individuals or families experiencing chronic homelessness through the coordination of appropriate services; to provide consistent and ongoing community-based support as needed by the person supported using an intensive case management approach; to provide responsive supports and services to the person's multiple and changing needs within the context of a trusting and productive relationship that assists the person to achieve goals, and stabilize his/her life; to embrace a collaborative approach to service delivery that provides hands-on, flexible and low barrier support that assists the person to maintain housing. This position works throughout the County of Simcoe. This is a contract position, working up to 37.5 hours per week until March 31, 2027.

Compensation Package:

  • Work up to 37.5 hours per week.
  • Minimum starting rate of pay is $34.51 per hour.
  • 11 paid statutory holidays.

Minimum requirements of this position include:

  • Community College Diploma or a University Degree in social services or other closely related field or a combination of education and experience which provides the required knowledge, skills and abilities as determined by the employer
  • Minimum of 2 years recent full-time work experience within a social services/health care or related environment, including providing case management to individuals/families experiencing chronic/episodic homelessness
  • Demonstrated knowledge of Housing First principles, guidelines and related housing concepts, acts and regulations
  • Knowledge of psychosocial rehabilitation, recovery principles and harm reduction philosophy
  • An understanding of case management; including assessment, service planning, skills reaching, monitoring and evaluation
  • Ability to engage sound judgment and strong problem-solving skills; excellent written and verbal communication with administrative and organizational skills and ability to prioritize tasks
  • Strong interpersonal skills with proven ability to work independently as well as part of a team
  • Demonstrated knowledge of human/legal rights and relevant legislation.

Duties and responsibilities (including, but not limited to):

  • Provide community-based case management to people experiencing chronic homelessness at a high level of acuity in the areas of, but not limited to: independent living skills, housing stabilization, money management, community integration, employment linkages, and community connection
  • Attend and/or accompany the person to community appointments for support, as appropriate
  • Assist them to maximize their options and potential by facilitating their involvement in community and social activities, recreational and training opportunities
  • Conduct comprehensive assessments of the person's needs
  • Assist in development, implementation and revision of the person's support plans, monitoring progress toward identified goals
  • Work with the person on a day-to-day basis in a professional, non-judgmental and discrete manner to facilitate the individualized support plan
  • Engage with the Housing First team to share acquired information related to the person supported to establish pertinent details of an individualized support plan
  • Conduct crisis management and risk assessments in consultation with supervisor and/or Housing First Team
  • Provide crisis management for individuals; making linkages for intervention, as appropriate
  • Advocate on behalf of the person supported to access necessary physical and mental health care, legal and financial supports
  • Conduct outreach to potential clients through community-based partners.
