About the role

Position: Home Share Coordinator – Temporary Part-time for 6 months with the possibility of extension.

Location: Cloverdale & Ladner, BC

Hours: 20 hours per week, with flexibility to work on evenings and weekends is necessary.

Salary: $27.81

Start Date: As soon as possible

We are looking for a Home Share Coordinator to join our Sources Community Living Services team! This individual must be passionate about working with people, engaging, and organized.

Description of Position:
The Home Share Coordinator is responsible for aiding the program manager in the recruitment, operation, support, and ongoing evaluation of the home share program. Our mission is to provide adults with developmental disabilities with a safe and stable home within the community. By carefully matching and providing ongoing support to our clients and caregivers, we can ensure that the home is stable, provides excellent care, and maximizes the quality of life of both the client and caregiver.

Reporting to: Home Share Program Manager

Responsibilities:

  • Participates in the development of program outcomes, the preparation of outcome reports, individual program plans and behaviour programs as necessary.
  • Ensures that appropriate goals are set for clients and that all members of the team, work towards reaching the goals of the individual program plans.
  • Liaises with natural families, caregivers and ministry/CLBC personnel, including coordinating regular review meetings; also includes ensuring all parties are informed of any changes or issues arising, as pertaining to the client's well-being.
  • Carries out home studies leading to the recruitment and training of caregivers, whom the coordinator selects and ultimately matches with clients.
  • Initiates, renews, and terminates contracts as needed.
  • Acts as a resource to the caregivers, the clients, and their families.
  • Apprised of Contractor’s scheduling of weekend visits and vacations of clients with natural families.
  • Travel to clients home in the community using your personal vehicle
  • Makes regular monitoring visits to family care homes and evaluates caregivers on the standards of care.
  • Ensures the Home Share program continues to meet or exceed the COA standards, the policies and procedures of Sources, our funding agencies, and our contracts.
  • Attends and participates in program meetings as an active contributor, bringing forth ideas and solutions to problems that arise.
  • Attends and aids in the facilitation of the Sources Surrey and Delta Social Group as directed.
  • Is available to clients and caregivers by phone for emergencies and any unusual situations.

Qualification Assets:

  • A degree or diploma in an area related to supporting people with developmental disabilities (Social Services, Human Services, or equivalent)
  • Effectively utilize Office suite including Excel, Word, Outlook and central data systems
  • Effectively communicate professionally, verbally and written in English
  • Empathy and good emotional intelligence.
  • Work with initiative and integrity.

Qualification Requirements

  • Valid driver's license
  • Clear criminal record check
