About the Role:

Coordinate and support the delivery of consumer-directed aged care, disability and home support programs that empower people to remain independent and connected to their communities. Working closely with consumers, carers and multidisciplinary teams, you'll oversee service planning, staff coordination, quality improvement initiatives and program performance, ensuring services remain responsive, person-centred and aligned with funding and compliance requirements.

About You: 

  • Experience leading teams and coordinating community-based services.
  • Strong communication, organisational and stakeholder engagement skills.
  • Demonstrated experience in aged care, disability, rehabilitation or community health.
  • Ability to manage program performance, compliance and continuous improvement activities.
