The Job The incumbent will report to one of the CYMH Service Managers and will work from the Bracebridge office. The purpose of the Service Planning Coordinator is to provide inclusive and culturally appropriate systems level service integration, coordination and support for children, youth and their families with complex special needs where additional case management and service co-ordination is required. Primary duties include: to provide leadership to a collaborative and integrated approach to service delivery by engaging the child/youth/family, professional support services, and community support services to work together on behalf of the child by documenting strengths, needs, goals and service strategies, establishing target dates for work to be accomplished and all is documented in a Single Plan of Care. This is a Bargaining Unit position CUPE Local 5319.
Qualifications
- College Diploma in Human Services or equivalent with some direct experience in child and youth work in a community setting.
- Proven knowledge and understanding of service coordination and effective system-based interventions related to children, youth and their families obtained by training and/or extensive experience in various capacities with this population.
- Extensive knowledge of services and resources available, both at a local and provincial level.
- Demonstrates knowledge of and successful application of relevant legislation (i.e. CYFSA, Human Rights, Health & Safety, PHIPPA) and an understanding of Ontario’s Special Needs Strategy.
- Ability to write clear & concise reports, computer literate, and able to meet deadlines and other administrative requirements.
- Ability to work as part of an interdisciplinary team and with communities.
- A thorough understanding of anti-oppression values and principles is important.
- Valid driver's licence and access to a vehicle is required.
- Bilingualism (English/French) is a highly desired asset.
Compensation $70,040 to $82,893, commensurate with experience.
