Position #: 186995 Expected Start Date: September 21, 2026 Union: HSAS Facility: Victoria Square City/Town: Prince Albert Department: Mental Health Community Case Management Type: Full-time temporary Expected Up to Date: January 03, 2028 FTE: 1 Number of Hours per Rotation: 112 hours per 3 weeks Relief: No Float: No Hours of Work: Field Hours Salary or Pay Band: Pay Band Assessor Coordinator - Degree $36.721 to $44.902 Travel Required: No Job Description: Reporting to the Manager of Mental Health and Addiction Services, the incumbent is responsible for the screening and coordination of initial service requests for all programs within Mental Health and Addictions Services in the former health region. The Intake Facilitator provides screening, triage, assessments for urgency/risk, and coordinates case assignments. The position facilitates crisis response and short-term support for services to clients presenting with a variety of mental health and addictions issues. The Intake Facilitator provides consultation to Community Agencies and Programs regarding services available in Mental Health and Addiction Services. Requirements
Candidates must hold a Bachelor's degree in Social Work, Psychiatric Nursing, Addictions Counseling, Social Sciences, or Nursing. A valid Class 5 driver's license and registration with the Saskatchewan Association of Social Workers are also required.
