The Calgary Foothills Primary Care Network (PCN) is a group of more than 600 family doctors and nurse practitioners who work with teams of other health professionals to offer patients the best in primary health care. Our PCN serves 400,000 patients in northwest Calgary and Cochrane. Together with our community partners, we offer round-the-clock care. We work to achieve better health outcomes for our patients. The physician will provide primary care services to patients with complex needs, including chronic conditions and mental health concerns. They will work within a collaborative team environment to manage patient panels and coordinate care.

The Mental Health Consultant provides single-session counselling to individuals, couples, and families while supporting patients with self-management and healthy lifestyle coaching. They also collaborate with the medical team to navigate health care resources and maintain accurate service data.
